Color Projected Image Division (CPID): Photo Essay Contest - Entry Form

This contest is available to individual members of PSA. A PSA membership number is required when entering, and a copy of the PSA Membership Card or a PSA Journal mailing label needs to be mailed with the entry.

Please enter me in the PSA CPID Photo Essay Contest. I will be mailing a CD or DVD with a copy of my PSA Membership Card or my PSA Journal mailing label to the Photo Essay Contest Director within two weeks.
